What is Community Nursing Care?
Community Nursing Care provides registered nursing services to NDIS participants in their home or community setting. Our experienced registered and enrolled nurses deliver clinical care that enables you to manage complex health needs without the need for hospital or residential care.
We specialise in supporting participants with chronic health conditions, complex medical needs, and post-hospital care. Our nurses work collaboratively with your GP, specialists, and support coordination team to ensure seamless, integrated care.
All nursing care is delivered by AHPRA-registered nurses with experience in disability and community health settings.

How We Deliver This Service

Our registered nurses conduct an initial assessment to develop a personalised care plan aligned with your NDIS goals. Nursing visits are scheduled at agreed times, with emergency support coordination available when required. We maintain detailed clinical notes and communicate regularly with your healthcare team.
